Application Information for Arizona Long Term Care
For individuals who are 65 or older, blind, or disabled and need ongoing services at a nursing facility level of care. Many participants live in their own homes or an assisted living facility and receive needed in-home services.
Area Agency on Aging
Provides a vast variety of programs and services to enhance the quality of life for residents of Maricopa County, Arizona. As a non-profit organization it advocates, plans, coordinates, develops and delivers services for adults aged 60 years and older, persons of any age with HIV/AIDS, adults aged 18 years and older with disabilities and long term care needs, and family caregivers.
Arizona State Braille and Talking Book Library
Provides books, magazines and other library resources in alternate formats for all Arizona residents whose visual or physical disabilities prevent the use of conventional print materials.
